We welcome new patients to the practice who live within our catchment area. To register, please either click the following links:

https://gp-registration.nhs.uk/G84003001 – if your home address falls under our Mottingham branch
or alternatively, you can register with the practice via the NHS App.
If you are unable to register via the above methods please come into the surgery and collect a registration form to complete or you can print the GMS1 and registration form from our website and once completed bring into the practice.
We will also need you to bring the following when you come in to register:
- Photographic proof of identification (eg passport, driving license).
- Proof of address not older than three months (eg bank statement, utility bill).
- History of vaccinations for all children aged six years or under.
